POSTCARE-O: Patient-Owned Survivorship Care Planning in Ovarian Cancer

Welcome to the Patient-owned Survivorship Transition Care for Activated, Empowered survivors Ovarian (POSTCARE-O) training!

The POSTCARE-O intervention is a health coaching process that comprises eight core components to facilitate the development of a patient-owned survivorship care plan. The goal of the POSTCARE-O intervention is to enhance survivors’ self-management, health outcomes, and health care utilization as they transition from active treatment to survivorship care.

This online course is designed for you to cultivate the skills needed to deliver the POSTCARE-O intervention in your work with ovarian cancer patients. We encourage you to take notes and reach out with any questions you have during your training. You can also post your questions in our interactive discussion board that is available to you. Your POSTCARE training team will answer your questions to maximize your learning.  

At the end of this course once all of your modules have been completed a member of our training team will reach out to schedule a time to complete your competency assessment via zoom. Upon completion of your competency assessment you will be provided with instructions on how to receive your CEU certificate as well as your POSTCARE-O facilitator training certificate.   Finally, you will be asked to complete a brief phone interview to get your feedback on the course and training.
